FEDERAL EMPLOYEE DISABILITY INSURANCE BENEFITS PROGRAM

The
Federal Disability Insurance Benefit Programs are available to ALL
Federal Government Employees, Non Postal Government Federal Employees
and All Postal Employees. The Federal Employee Disability Income
Insurance Benefit Program is purchased in $100 per month increments up to
$2,500 per Month. The minimum Monthly Benefit is $600 and the Maximum Benefit
can not exceed 66 2/3% of an Employees GROSS Pay or $2,500 Per Month, whichever
is less.
During OPEN SEASON Federal
Disability Insurance Benefit Program is GUARANTEED TO ISSUE with No Riders
or Waivers and Covers Any Pre-Existing Condition(s) after 12 Months Coverage
under theProgram Plans.
All
Federal Employees actively at work full time at their regular Government
Occupation for 30 days Prior to Enrollment, CANNOT BE DECLINED for
Benefits or Coverage. NO MEDICAL CONDITION(s) WAIVED OR RIDERED OUT of the Plans
Coverage. ALL PRE EXISTING CONDITIONS ARE COVERED By Plan Benefits after 12
months. Federal Employees Must Enroll During OPEN SEASON to be
eligible for the Federal Employee Preferred Risk Underwriting .
Federal Employees select Short Term Disability Income Insurance Plan
Benefits to start Monthly Benefits on the 15th or 31st Day (1st day if
hospitalized) of a covered Disability. 24-Hour Coverage On or Off the Government
Job Included. Federal Government Employees Select Short Term
Disability Income Plan BenefitsPeriod to be Payable for ONE Year or TWO
Years. The Disability Income Benefit Period Increases during the First Five
Years. After Five Years, A ONE Year Disability Plan Benefit Period
Increases to Eighteen Months. A Two Year Disability Plan BenefitPeriod
Increases to THREE Years with NO Increase in the Disability Program Plans
