If you lose or have a tooth extracted, Smile Dental Plan provides coverage for the lost tooth and ANY adjacent teeth even if missing BEFORE your Effective Date. Smile Dental Plan will Waive the five year replacement ruling & Pay Benefits toward a New fixed bridge, partial or denture. The Dental Insurance will ALSO PAY Benefits for Teeth Missing or Lost BEFORE your Plan Effective Date after being insured for thirty six months. None of the Federal OPM, FEHB, FEDVIP, BENFED Dental Programs will Provide these Benefits.
